Our Survive-to-Thrive coordinator will help you find the resources you need to survive and connect you to programs and services that will help you thrive. Not only will you leave with a plan, but you will also have easy-to-follow action steps to remove the stressors and barriers to a thriving life.
INTAKE FORM
Please complete our online intake form before your appointment. Please come in 15 minutes before your meeting to complete it if you need assistance with the form. The intake form will ask you to sign a release of information (ROI) form. This ROI is not required, but it will help us provide you with the most comprehensive care. In addition, the ROI form enables us to share basic information with other programs and agencies to find the best help and resources for you and your family.

What to Bring

If you are meeting with our Survive-to-Thrive Coordinator or seeking financial assistance of any kind, please bring the following items to your appointments.

Identification

Copy of Bill

IDENTIFICATION

State IDs, birth certificates, and social security cards can be applied for in the initial appointment. Bring what you have.

PROOF OF EXPENSE

This could be a copy of a bill, quote/estimate, lease agreement, etc.

PATIENCE

Financial assistance is not guaranteed. We work with several agencies to help you find financial help. We never offer cash or immediate financial assistance.
